Going camping? You need to fill out the medical form

form
Download the form

If you’re going on the campout this weekend, you need to provide a current BSA health and medical form for your scout at check-in.

Please fill out and print the form and bring it with you to the park. A limited supply of extra forms will be on hand if you forget, but please try to bring a completed form to avoid delays and keep the check-in flowing smoothly.

This information is necessary to ensure your son’s safety and is required by BSA rules.

Please direct any questions to Committee Chair Bart Alcorn or Cubmaster Lance Wallace.
